After completing the RMA form, email a copy to orders@destinysolutions.us and include a copy in your package being shipped to us.

WE CANNOT ACCEPT YOUR REPAIR UNLESS THIS FORM IS COMPLETED AND INCLUDED IN YOUR PACKAGE.

All shipping costs for warranty and non-warranty repairs will be incurred by the customer.
